Application forms
On this page you will find all of the relevant application forms for crop and livestock enterprises (plus livestock markets). There are some forms that all applicants must complete, plus additional ones, depending upon your business. We've spelled out the mandatory forms for each enterprise, for the sake of clarity, and you can download all necessary documents from the lists below.
All of our application forms are in Adobe PDF format (click here for more information if you are unfamiliar with this).
- Crop producers (arable, field veg and horticulture) must complete the Application Form (RD 254), the Producer Conversion Plan (RD 92), the General Information form (RD 221), the All Crops form (RD 226) plus any additional sections relevant to your business from the list below. All applicants must also complete the Declaration and Licensing Agreement (RD90).
- Livestock producers must complete Application Form (RD 254), the Producer Conversion Plan (RD 92), the General Information form (RD 221), the General Livestock form (RD 227) plus the additional sections from the list below relevant to the livestock enterprises on the farm. All applicants must also complete the Declaration and Licensing Agreement (RD90).
Each question is linked to the relevant section of the Control Manual (our organic standards document) and by completing the application forms, all the relevant sections of the Control Manual will have been referred to and a comprehensive conversion plan will have been produced, as is required in the regulations.
