Application forms
The OF&G Application Form for farmers, growers and livestock producers is available as a series of Adobe PDF files (click here for more information if you are unfamiliar with this format). Each section covers different production enterprises and can be viewed by clicking on the appropriate heading.
- Crop producers must complete the Application Cover, Application Form 1 General and Application Form 2 Crops.
- Livestock producers must complete Application Cover, Application Form 1 General and Application Form 3 Livestock General, plus the additional sections relevant to the livestock enterprises on the farm.
- Each application MUST be accompanied by a signed copy of the Declaration & Licensing Agreement (RD 90) - see below.
Each question is linked to the relevant section of the Control Manual and by completing the Application Form, all the relevant sections of the Control Manual will have been referred to and a comprehensive conversion plan will have been produced, as is required in the Regulations.
